I consent to the agency using de-identified information about my care (no name, address, contact details, or exact birthdate) with a contracted zero-retention AI service to help draft care summaries and family updates, which staff always review before use. This feature is optional; declining does not affect my services. AI assistance is OFF unless both the agency enables it and this consent is granted.
