Consent to Personal Support Services

I consent to receive personal support services from the agency. I understand the nature of the services described in my care plan, that I may refuse any service at any time, and that my personal health information will be collected, used, and stored in Canada solely to plan and deliver my care, in accordance with applicable privacy legislation (including PHIPA in Ontario / PIPEDA where applicable). This consent remains in effect until withdrawn.
